The pictures show different types of holders, the installation procedure is done the same way for all holder types:
Remove the screws in the holder (NOT the center screw). The screws are to be replaced with similar enclosed longer screws. Remove the tilt swivel from the holder. Place the top support between the holder and the tilt swivel so the adjustment screw is on the back and the holes fit. Place the enclosed long screws in the holes and screw the holder onto the tilt swivel so the top support is locked into place.
